Food Acknowledgment & Assumption of Risk

I acknowledge and agree to the following on behalf of myself, any minor children under my care, and any individuals (including family and friends) with whom I may share or serve the food provided. I understand that raw and/or cooked food items will be distributed at this event. This may include, but is not limited to: turkeys, produce, prepared foods, beverages, snacks, and other edible items.

I understand that:

  • All food is provided at no cost, “as-is,” and without any guarantee or warranty (express or implied) regarding quality, freshness, storage, preparation, or ingredients.

  • Some food may be raw, perishable, or require proper handling and cooking to prevent illness.

  • I am solely responsible for the safe transport, storage, preparation, handling, and consumption of any food I receive or choose to share with others.

  • I am aware of risks including but not limited to foodborne illness, allergic reactions, contamination, choking risks, and other health-related conditions.

By accepting food at this event, I voluntarily assume all risks associated with the consumption and sharing of the food with myself, minors, and any third parties, and release T.E.A.M. Inc and all affiliated/partnered organizations, volunteers, sponsors, and representatives from any liability or claims arising from the distribution, use, or consumption of the food provided.

Services Acknowledgment & Assumption of Risk

I understand that free services (including health screenings, dental cleanings, and community support services) may be offered by licensed or volunteer providers. I acknowledge and agree that:

  • Services are voluntary and provided as a courtesy to the community.

  • No guarantees are made about the outcome or accuracy of any service provided.

  • Any medical or dental recommendation is not a substitute for professional ongoing care.

  • I am responsible for any follow-up or treatment decisions.

Release of Liability

By signing below, I voluntarily release and forever discharge:

T.E.A.M. Inc, Empire Dental, all event partners, sponsors, affiliates, volunteers, employees, and agents

from any and all liability, claims, or demands arising from:

  • Illness or injury related to food provided

  • Improper food preparation, storage, or handling

  • Allergic reactions

  • Participation in any services offered at the event

  • Negligence or acts of third parties

This waiver applies to me and to any minors and any individuals (including family and friends) with whom I may share or serve the food provided.

Indemnification

I agree to indemnify and hold harmless the above organizations and individuals against any claim, loss, or expense arising from my participation or use of provided food or services.
